MLT ASCP Routes: Are You Qualified?
Do you know which MLT ASCP routes demand applicants to submit the experience documentation form?
It is route 4 for United States candidates and route 3 for international candidates.
Upon meeting the eligibility requirements of routes 1 and 2, the non-U.S. applicants also have to submit the training documentation form.

MLT ASCP Routes: For United States Applicants
Route 4 Prerequisites
- Three years of clinical experience in 6 six areas (microbiology, hematology, chemistry, immunology, urinalysis and other body fluids, and hematology) from an approved lab
- Associate certificate with the combination of six semester hours in biology and chemistry
- Or Academic credits 90 quarter hours with 9 quarter hr in chemistry and 9 quarter hr in biology.
Route 4 Experience Documentation Form
If you are satisfied with route 4 prerequisites in all MLT ASCP routes, you have to fill out the experience doc agreements to meet this ASCP certification eligibility requirements.
The following is all the information about the Path 4 Experience Documentation Form:
Part I
It is related to applicants, and it must be completed by them.
It requires the applicant’s
- Name
- Email address
- ASCP customer ID#
- City, state, zip code
- Last 4 digits of social security#
Part II
To make the form acceptable, part II must be completed and signed by the immediate supervisor or lab management.
This part is divided into sub-sections, which are
- This section requires information on the program’s start and end date.
- Here, the program director needs to review the experience of applicants. To be form completion, one is responsible for proving his/her competency in simple and complex tests including post- and preanalytical components in all the areas that are given below:
- Blood banking
- Microbiology
- Chemistry
- Immunology
- Hematology
- Urine and Other Body Fluids
- In this section, the lab management or program director verifies the applicant’s experience by signing this form by inserting the following information:
- Name and degree
- Signature
- Institution
- Telephone number
- Title
- Date
- Email address
- Zip code
MLT ASCP Routes: For International Applicants
Route 3 Prerequisites
- Chemistry/biological science 2-year diploma
- Three years of clinical experience in 4 areas (chemistry, microbiology, hematology, and blood banking)
Route 3 Experience Documentation Form
From all MLT ASCP routes that the board suggests for foreign lab technicians, if you meet the route 3 requirements, you have to submit an experience documentation form.
This form also comprises two parts: part I for the applicant and part II for the lab manager/employer.
Part I
It is related to the applicant’s personal information:
- Name
- Email address
- Address
- ASCP customer ID#
Part II
It must be accomplished by the laboratory management or employer.
It verifies the applicant’s experience for exam eligibility.
Like the U.S Route 4 experience documentation form, it is also categorized into three sections.
- Explain information about the applicant’s experience starting and ending date.
- Shows the area of experience that the candidate must be approved by the employer or lab management. There are four areas for international lab technicians:
- Blood banking (Immunohematology)
- Hematology
- Microbiology
- Chemistry

Special Note for U.S and non-U.S Exam Hopefuls
Never forget to include a letter of authenticity from your employer or laboratory management with a paper of experience doc.
It must be printed on the original letterhead.
The following state must be available on it:
This experience documentation paper was dated, completed, and signed by your lab management/program director/employer.
Without a letter of authenticity, the experience documentation form is unacceptable.
