ASCP MLT Pathways: Approved Laboratory

The education that you received from the ABHES/NAACLS-authorized MLT program is only workable for 5 years after the date of finishing this program.
To complete your online application form, you must upload the following docs in PDF format.
- Official copy of your SBR, EBR, or DD214 form
- Official credential verifying your training program completion
- Official papers of your military code (MOS, NEC, 68K, or AFSC code)
