  • Associate degree with 6 semester hours in chemistry & biology from any recognized institution
  • Academic credit sixty semester hours with six semester hours or 9 quarter hours in chemistry & biology from an accredited institution
  • Within the last 10 years, completed a 50-week U.S. military medical lab training program.

MLT ASCP Documentation

ASCP BOC offers two documentation forms for Route 4: The experience documentation form and the MLT or MLS program completion documentation form. Both the supervisor and the candidate must fill out and sign both of the documentation forms provided by ASCP BOC for Route 4.

Experience Documentation Form for Route 4

This documentation form has 2 parts: one for candidates and a second for supervisors.

Part I is for candidates and they should complete yourself. The information you need to provide there is the applicant’s name, Customer ID of ASCP#, email address, last 4 digits of the candidate’s social security #, city, state, zip code, and address.

Part II form must be completed by the laboratory management and immediate supervisors to make it. The supervisor must provide the required details about the candidate's work history to show their eligibility for the exam.

All post and pre-analytical components of tests, including those in blood banking, chemistry, hematology, microbiology, immunology and all body fluids, must be familiar to candidates.

Candidates must submit this experience documentation form with authenticity letter from the lab management or their direct supervisor. It must be clear that your lab management and supervisor completed, signed, and dated this documenting form.

MLT Program Accomplishment for Route 4 (Not for Route 1)

The MLT program completion documents can only be submitted by candidates who have one year experience.

For both supervisors and candidates, it consists of two separate sections. Here, the candidate and supervisor must carefully include the candidate's personal, professional, and educational information.

Part I required the applicant’s name, email address, last 4 digits of the candidate’s social security#, city, state, Customer ID of ASCP #, zip code, and address.

The program director must sign, verify, and complete Part II with states that the applicant successfully finished the required program. Institution name, city, state, and types of programs with starting and completion date are required information of part II in document form.

Program director name & credential(s), title, program director signature, date, telephone number, email address, city, state, and zip code are required information related to the Program director in part II.

MLT ASCPi Requirements for International Certification

For overseas candidates, BOC offers 3 options (route). You can use these routes to determine your eligibility if you wish to obtain an MLT certificate.

MLT ASCPi Route 1

  • Medical Laboratory Science two-year diploma (certificate in medical technology, biomedical laboratory science, clinical laboratory science, and medical laboratory science) and with training in hematology, immunohematology, chemistry, and microbiology from a recognized technical institution

MLT ASCPi Route 2

  • 2-year diploma of biological science or chemistry from accredited/approved institution
  • Completed degree of Medical Lab Science with the training program of microbiology, hematology, immunohematology, and chemistry

MLT ASCPi Route 3

Routes 1 and 2 Training Documentation Form

This documentation also has 2 parts same as others. Part I for the candidate and Part II for program director.

The student’s name and program name are requirements for part one. Candidate should enter this information in the documentation carefully.

For approval of this documentation forum by BOC, part II must be covered by the program director.

In part two, program director verifies the candidate's eligibility for MLT ASCPi by entering some information including verification of clinical training of four subjects including immunohematology, hematology, chemistry, and microbiology as part of their academic program, length of clinical training program, and date of completion.

Route 3 Experience Documentation Form

Part I required the applicant's personnel information including the applicant’s name, address, email address, & customer ID of ASCP#.

For the Part 2 form, the employer or lab management must sign and complete it. It required laboratory management name & credential(s), title, laboratory management signature, date, telephone number, zip code, state, city, institution, and Email Address. Lab management confirms the candidate's eligibility for MLT ASCP by providing information such as experience of clinical training in 4 subjects, including immunohematology, hematology, chemistry.

Special Note for All Forms

Make sure to submit this experience documentation form with authenticity letter from your immediate supervisor. It must be mentioned that your direct supervisor or laboratory management filled out, signed, and dated this experience documenting form.
